Change Data Tier for recurrence_tumour_staging_system field

Implementation:
Based on discussions with MOHCCN Clinical Steering Committee and ARGO CMD Working Group, the following fields in the Follow Up schema will have their Data Tier changed from "Core" to "Extended":

• recurrence_tumour_staging_system
• recurrence_t_category
• recurrence_n_category
• recurrence_m_category
• recurrence_stage_group

Rationale:
According to several pathologists, recurrence staging is supposed to be done even for late-stage cancers, but almost all institutions skip doing this.
