Comparing http://tools.wmflabs.org/hashtags/search/oabot?lang=en and https://en.wikipedia.org/w/index.php?title=Special:RecentChanges&tagfilter=OAuth+CID:+817 , it seems that some edits are missed by the tool, and I can't figure out why. Any idea?

There's another tool (hatnote/hashtags) that monitors recent changes for edits with tags, and builds the database used for this tool.

One possibility: does this revision match this pattern?

(^|[[:blank:]]|\\.|\\!|\\/)[#＃][[:alnum:]]+[[:>:]]

Many of the missing edits have identical summaries to edits that are caught.
Example:

Same edit message:
Added free to read links in citations with [[WP:OAbot|OAbot]] #oabot
