Image Occlusion cards don't add

Problem description

I have had image occlusion w no problems for a year or so now. Recently, I realized that when I add new cards, they never actually appear. Not in browse, not in another deck, they just disappear. I have updated Anki, checked for IO updates, detected and redownload IO, copied and pasted the fields code, done literally everything. When I "add the cards", Anki says " one card added", but no new cards appear anywhere. The card just disappears
