Add issue templates

I think it would be very useful to have issue templates, because a large portion of the issues created are for help with levels. I think it would be helpful to have templates for:

• Bugs
• Help
• Feature requests

The templates wouldn't even have to be very elaborate. The main benefit would be automatic tagging so the issues can be easily searched/filtered
