Paragon actions should not be shown by default on a monster sheet when the monster has legendary actions. Showing both sets at once can be confusing to users.

Item collections can now be collapsed/expanded.
Full update of item styles and layout.
Items are now collapsed by default.
Added common logic handlebars helpers.
Also addresses:

- [#37] Expand/collapse item descriptions by clicking.
- [#38] Add roll button/action to item portraits.

[#38] Don't show paragon actions if legendary actions are visible

Paragon actions are now hidden if legendary actions are visible.
Checking "always show" will force paragon actions to be visible.

Fixed in v1.0.0. Paragon actions now hidden when legendary actions are active.
