Patient Proxy is a web app that helps users generate living will documents.
This was my first major undertaking in PHP web development. I did not use any php framework. I used maphilight to make the map of the US highlight and selectable; I used Stripe to handle payments; and I used fpdf to help generate the output pdf at the end.
Unlike everything else I do, this was for pay, not just for fun. Please do not use any of this without contacting me first! It is all under copyright, but is here for employers and friends to glimpse.
