Sorry but I cannot reproduce this one. If I understood well, you pressed attachment icon, went to the system file selector and came back (after cancelling or selecting a file?). Is that correct?

repro video on my iphone 12 max:

just open keyboard, go into album picker, press cancel: borked layout.
