Tested on master

We should normalize the way this looks and keep it consistent across all products. All products currently contain this information in a <EuiPanel> but APM does not.

The name, output, etc UI should be contained in a panel.

@chrisronline I see a few tiny inconsistencies, but maybe it would be good to update this issue with a checklist so whomever gets to it fixes the right thing(s)? I'm not sure I'm seeing the same things you're seeing. :)
