[Feature Request] Book request submission form

Is your feature request related to a problem? Please describe.
While CWA has helped a lot with automatic ingestion, the process of receiving requests and obtaining the epub has always been a manual one (via text, chat, you name it).

Describe the solution you'd like
A button on the main page which opens up a form for users to submit book requests and puts this information somewhere accessible for the admin. Alternatively, if email is configured, email the list to the designated address. This will help the admin to automate the search and obtaining of such books. For bonus user experience points, explicitly point/show this button when search results return empty.
