Allow optional table headers to be added when using AssociativeList #33

Sometimes, folks want their fields to be arranged vertically, and they would still like to have column headers, with Key and Value or Name and Data or something like that at the top. This could easily be achieved with some metadata in the FormatterOptions.

AssociativeList has been renamed to PropertyList. It is not yet possible to add a header row to the table via metadata.
