How to facilitate a standalone form to get giftaid declaration? #78

For a year and a half, we have not taken any gift aid declaration due to mismanagement of the profile setup. But I would like to contact those donors now and receive a declaration from them so we could claim gift aid on those donations. Just wondering what would be the best approach to achieve this? I have tried to use checksum & contact id method by giving the option to edit the profile, but that throws an error. Any help would be very much appreciated. Thanks.
