Overview

New Contact Us page design needs to be updated to mimic the existing contact form fields. (http://civictechindex.org/about/contact) Asterisk marks a required field.

Action Items

  • New Contact Us page should include the following fields
  • Email*
  • Name*
  • I have a question, comment, or feature suggestion*
  • Organization / Brigade / Affiliation
  • Your Project or Organization's GitHub URL

Resources/Instructions

Field inventory should match as seen below. Required fields will change as noted above.

future of this issue is creating a connection with backend. Need backend capacity for this

The git branch 'reimplement-contact-form' will get this form to display on the site page '.../contact'. However this version of the form is a component that is already merged into the main branch and is available under Contact/ContactForm.js
