This will need a further investigation.

Recently I have observed missing index in a couple of instances recently upgraded to 3.9 (including getting the latest code of tool_trigger).

Table: tool_trigger_run_hist. Index: not unique (stepconfigid)
CREATE INDEX mdl_tooltrigrunhist_ste2_ix ON mdl_tool_trigger_run_hist (stepconfigid);
