When pressing a key such as Shift+NVDA+U, the Image description reding often starts from the last line, or a point in the middle of the description. Expected behavior: The image description reads from the beginning. It would also be great to hear the description without the window title "image description" spoken as it would be a faster result. This is especially amplified now that 4O yields responses in about 2 seconds.

Love this suggestion. In the meantime, if your response does not have structured data, what I often do is disable the browsable dialog option in settings and review it with speech history (if needed).
On cursory investigation, it would appear that NVDA's browsable message code uses the old and outdated MSHTML ShowHTMLDialogEx function. You will notice that the same happens when triple pressing NVDA+f. This will likely take some time and consideration to implement. If nothing better can be found, we could open up the result in a readonly WX text control.
