adding anchors to h3 headings causes TOC to stop scanning subsequent h2 headings #188

Canonical support & MAAS users depend on anchored headings; h2 is adequately anchored in TOC, but if i do the following:

first h2 heading

First h3 heading

second h2 heading

Then TOC only reports "first h2 heading" and doesn't process any further h2 headings.
