Forms Updates for Form 45 Decal Replacement

1. There’s no mention of the fee or method for submitting. This may benefit from some brief instruction to advise of the cost of $10.00.
2. The office use space is inconsistent across MHR forms. It would be ideal to establish a consistent design that could be employed on all MHR forms.
3. Section 3 is titled mailing address of submitting party but is actually capturing the name and address so could be more appropriately labelled. Perhaps simply Submitting Party.
4. Section 3 does not include a space for submitting party email, or break down the address into the fields in the system. If the client is required to enter specific fields (i.e. street address, city, postal code, country, delivery instructions) they may be more likely to include all address components). I’ve frequently seen free formed addresses missing information.
5. In section 1: "Mailing Address of Owner(s)"
6. In section 2: "2. CURRENT LOCATION OF MANUFACTURED HOME"
7. Update declaration statement to remove plural references. For instance "I declare..."
8. Consider update to add text in section 5 for decal required to read "Indicate one or both". This helps clients know they may select an interior decal, exterior decal, or both.
