SNMP trap management page (Riak EE only) [JIRA: RIAK-1726]

Provide an SNMP trap management page (Enterprise only).
Graphical interface to set thresholds for the various SNMP traps that were implemented for 1.3 by Steve Vinoski (http://docs.basho.com/riakee/latest/cookbooks/SNMP-Counters/ ). These thresholds are currently set in the app.config and the relevant snmp config file, for each node.
It would be extremely helpful to be able to manage the thresholds for all the nodes off of a single page (since they're going to be the same anyways).
