  • GenerateTop() implementation moved to SqlServer
  • GenerateLimitOffset() generates ISO SQL:2008 by default, overridden by SqlServer (and Sqlite)

Note the different between FETCH FIRST and FETCH NEXT... Doesn't seem worth DRYing...

One line difference was reason for DRYing
