Make instrumentations compatible with latest SDK

Opentelemetry released a new version of the base instrumentation and SDK. I would like to help but I know very little about how these packages work. Instead, I will leave the PR from the contrib packages as a reference:

open-telemetry/opentelemetry-js-contrib#1054

Thanks, contib packages are bumped in github but not yet published to npm.
I need to wait for @opentelemetry/contrib-test-utils to be published and then update this repo as well. Hope it will happen tomorrow
