Is there a way to create a multi-selectable drop-down list with a select element that has multiple attribute?

At that point it isn't a normal select with in terms as UX or UI. Our goal is to create the best select replacement that replicates all of the functionality. We don't want to add any "extras" because it can cause confusion or usability issues for screen readers.
