Help wanted: Code of Conduct distribution

Hello Qiskitters, we need your help with a treasure hunt of sorts!

As you may be aware we have a single source of truth for our Code of Conduct that lives here, and all other Qiskit repositories should have a CODE_OF_CONDUCT.md file that links to this (for example see qiskit-terra’s. While the Qiskit teams do their best to keep the ever-increasing number of repositories up to date, sometimes we miss things, so we would appreciate some help from our lovely community in finding any gaps that we may have overlooked. If you would like to help out you can do the following:

a) Go through the Qiskit and Qiskit Community Github organisations and find any repos that are missing a CODE_OF_CONDUCT.md or has one that is outdated (all of them should redirect to the single source of truth, like qiskit-terra does)
b) Open a PR to add/change the CODE_OF_CONDUCT.md , you can use this as an example (please follow the wording exactly)
c) Let us know in a thread here about any that you’ve found (even if you don’t have time to submit a PR for them letting us know what you’ve found is still super helpful!)
