Set up Qiskit bot to track contributor monitoring

We want to know how many issues and PRs are from GitHub users who are not members of the Qiskit GitHub organization. I think Qiskit SDK already does this, so it might only be a question of config.

The bot should add a label called 'community'. It's probably better to have a single label for PRs and issues.
