Document the Book creation process

For your TODO list (hopefully in the near future!): Add a chapter (or possibly more) to the documentation so that I know how to actually compile a book and update a configuration file. The source for the documentation is in OpenDSAX/Doc/source. Just add a line to index.rst for any files that you add, and write your chapter in a new RST file there.
