The ask of this ticket is to add a field that says “How ethical are the company’s training data collection practices?”

The choices should be:

Dropdown (6 options)
Default: Can't Determine

Good,
Average,
Needs Improvement,
Bad,
Not Applicable,
or Can’t Determine

There should also be another field to accompany this called “AI Ethical Explanation”. Which is a text input field.
