Allow download into hierarchy specified in the manifest

Currently, the only options are to download flat list of files into the destination directory, or download into the hierarchy that can be from the pre-defined choices. This can be too limiting in some situations, such as for NLST, where dates are non-empty and important, and a manifest that uses that information may be generated outside of idc-index.
