Create New Patient checkbox on Patient Check In stays checked on subsequent checkins

Expected behavior:
The Create New Patient checkbox should always reset to not be checked on new patient check in
Actual behavior:
If the Create New Patient checkbox is checked on patient check in, it will stay checked for the next patient check in
Steps to reproduce:

1. Go to Patients/Outpatient
2. Click on the Patient Check In button
3. Check in a new patient, making sure to check the checkbox labeled Create New Patient is checked
4. Go back to Patients/Outpatient
5. Click on the Patient Check In button
6. Observe that the Create New Patient is checked when it should not be checked.
