Add a flag to allow custom forms for a specific object to always be added

I would envision this being a check-box on the "New Custom Form" page. If checked, this would add the form each time the associated object (ie. patient) page is displayed. That would mean that if objects were created before the custom form, the new form would be "added" to those existing objects. Exactly how that would happen, could be a point of discussion.
