Improvement - Give meaningful names to report files

Expected behavior:
It would be nice if report files would have meaningful names, representing the content they have.

Actual behavior:
All exported report files are named: report.csv
After downloading a few reports, someone can be confused about their content, unless they are opened.

Steps to reproduce:
Go to Patients > Reports
Select Report Type and Dates.
Click 'Generate Report'
Wait for report to be ready
Click 'Export Report'
A CSV file is downloaded with the name report.csv

Try downloading any other type of report, it will have the same name.
