line 85:
input name should be:

$output .= '<input type="text" name="gk-reservation-size" class="gk-required" placeholder="'.__('Party Size*', 'gk-reservation-steakhouse').'" value="'.$size_value.'" />';

instead of

$output .= '<input type="text" name="gk-reservation-subject" class="gk-required" placeholder="'.__('Party Size*', 'gk-reservation-steakhouse').'" value="'.$size_value.'" />';
