Clean up the booking form to align with the new create event form

Depending on results from the usage of the new event form and community feedback on it. We should update the booking form flow to account for any simplifications. Especially around not needing ticket type drop downs, simpler "booked out" logic and messaging. Etc.

Early suggestion

• Delete any mention of waitlist, since there never was any
• Remove dropdown "to choose ticket type" as there are no types anymore
• Review booked out logic as it should be simpler now
• Possibly finally add in a contact form on the dojo page to allow people to easily contact club when event is booked out. This could be tweaked to replace waitlist logic if you have a "quick accept" button in the email to the champion for example.
