I prefer the first option, cleaner and gives the idea of buttons to click.
I would keep the search bar with dropdown though. It's redundant but it makes it easier to access questions by typing. Buttons can only be a certain number, we don't want to clutter the interface with hundreds of buttons. I'd display only 5-7 suggestions per time (depending on size of buttons).
