QCEvaluation needs a created: AwareDatetimeWithDefault field #1177

We need to track when QCEvaluations are created so that if a user appends an identically-named evaluation it "overwrites" any earlier ones.

In the portal we can have a UI element that allows users to go back in time if they need to see older versions of an evaluation.
